Benton County restaurant inspections: Turns out duct tape is not the answer to every problem | Northwest Arkansas Democrat-Gazette

Violations marked as priority contribute directly to the elimination, prevention or reduction in the hazards associated with foodborne illness. Priority violations include prevention of contamination, cooking, reheating, cooling and handwashing.

Priority foundation rules support, facilitate or enable one or more priority items.

Priority Foundations Violations: Drinks and food on floor of backroom and drinks on floor across from coolers.

Core Violations: Below backroom fridge had large amount of spilled food residue.

Priority Violations: Hot water heater not working.

Priority Violations: Container of rice noodles stored below pans of raw meat. Items on the buffet tables that are on a grate in the bottom of the pan are not at 135°F or above. Tomatoes, boiled eggs, cantaloupe, honeydew on the salad bar are not being held at 41°F or below.

Priority Foundations Violations: The drop guard inside the ice machine is cracked and not easily cleanable. Water is dripping from the drop guard onto the ice, possibly contaminating the ice. The spring cord on the ice machine is covered with duct tape. Duct tape is not an approved repair material.

Core Violations: Employees working with open food are not wearing effective hair restraints. Interior, doors and door handles on the refrigerator units are dirty.

Priority Violations: Empty soap dispenser in bar sink in front of TVs.

Priority Foundations Violations: Raw beef paddy stored above ready to eat tostados in cold hold, and raw meat stored above soup, and butter in walk-in. Raw meat stored above beer in bar walk-in.

Core Violations: No employee with food safety management course certification. Broken freezer door and ice buildup behind bread in walk-in cooler.

Priority Violations: Raw shell eggs stored above shredded cheese in walk-in cooler; raw beef stored on top of avocados in walk-in cooler. Fajita beef being held at 106°F and fajita chicken being held at 109°F in hot holding.

Priority Foundations Violations: No hand cleanser available at handwashing sink near ware-washing area.

Core Violations: Raw beef thawing in a container at room temperature with no running water. Accumulation of metal fragments around blade and piercing parts of can opener. Accumulation of black growth on plastic ceiling surface on interior of ice machine. Pipe beneath handwashing sink near ware-washing machine broken off and lying in floor below sink.

Core Violations: No certified food safety manager. Large pots, trays and other containers for food contact use were stored outside the back door of the restaurant. Cracked tile/flooring around dish area and hand wash sink allowing for water to pool.

Priority Foundations Violations: Raw meat stored above tortillas in walk-in freezer. Meat stored on the floor of the walk-in freezer.

Core Violations: No registering sanitizer in ware washing sinks. No sanitizer test strips for sink. No temperature measuring thermometer for dish machine.

Priority Violations: Chicharrones in hot hold unit were at 70 degrees. Packaged salsa, guacamole and peppers made in the facility were not date marked.

Core Violations: Holes above bathrooms, and leading to ventilation in store area. The back door doesn't seal properly. Small live roaches found on premises, along with larger dead ones. No test strips for sink. Restaurant walk in, and kitchen area relatively dirty, with dirty rags, and food residue buildup on racks, floor, counters and equipment.
